Appointments are typically made on a referral basis only. Please provide the name of your referring dentist or physician when calling to schedule an appointment so that we may contact your doctor’s office and obtain any essential documentation.
Our schedulers can be reached 8:00 a.m. – 4:30 p.m. Monday through Thursday and 7:30 a.m. to 3:30 p.m. on Friday at (309) 682-1213.
Patient Registration
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
